ABSTRACT

Introducción: El carcinoma verrucoso o tumor de Ackerman es una rara entidad relacionada con la infección por el virus del papiloma humano, comportándose como una variante menos agresiva del carcinoma epidermoide. Caso clínico: Presentamos el caso clínico de un paciente de sexo masculino de 45 años portador de una gran lesión perianal con compromiso del canal anal y el aparato esfinteriano. Discusión: Existen varias alternativas para el tratamiento del tumor de Ackerman. Sin embargo en casos como este, y sobre todo ante la ausencia de respuesta al tratamiento de quimioradioterapia, se requiere de una resección amplia y radical. Pese a ello, la recidiva es frecuente. Conclusiones: En el caso analizado se destaca la forma particularmente agresiva de esta patología, con extensa invasión local y pobre respuesta al tratamiento oncoespecífico inicial.


Introduction: Verrucous carcinoma or Ackerman's tumor is a rare entity related to human papillomavirus infection, behaving as a less aggressive variant of squamous cell carcinoma. Clinical case: We present the case of a 45-year-old male patient with a large perianal lesion with involvement of the anal canal and sphincter. Discussion: Ackerman tumor treatment admits different therapeutic modalities; however, in cases such as this, and especially in the absence of response to chemo-radiotherapy treatment, a broad and radical resection is required. Despite this, recurrence is frequent. Conclusions: The particularly aggressive form of this pathology, with extensive local invasion and poor response to the initial oncoespecific treatment, stands out in the analyzed case

ABSTRACT

Abstract: Vulvar cancer accounts for less than 1% of malignancies in women. Verrucous carcinoma of the vulva is a rare histological variation, comprising less than 1% of vulvar cancer cases. Although it is characterized as being locally invasive, the condition is not associated with metastatic spreading. Lesions present in the form of a verrucous, ulcerated, and bleeding tumor that can reach large dimensions. This type of tumor can be mistaken for condylomata, both macroscopically and microscopically. We report the case of an 81-year-old patient with a large vulvar tumor presented for eight years, initially considered as a Buschke-Löwenstein tumor. The patient underwent radical vulvectomy with a V-Y advancement flap technique. This type of tumor should be considered by clinicians dealing with condylomatous ulcerative lesions that do not respond to the usual treatment.

ABSTRACT

Oral verrucous carcinoma (OVC) is a distinct low grade variant of squamous cell carcinoma (SCC) of the oral cavity. OVC is a rare tumor representing only 3-4% of all carcinoma. OVC is characterized by a slow growing, painless, broad based verrucous or wart like papillary lesion. Th e lesion lacks ulceration and has a propensity for local invasion rather than metastatic spread. Transformation of VC to anaplastic or poorly diff erentiated SCC is about 30-40%. Surgical excision with adequate margins of resection seems to be the clear preference for treatment. Th e need for neck dissection is an important consideration in planning therapy for OVC. Th e aggressive clinical presentation of the tumor often sways clinical judgment in favor of performing lymph node dissection, especially in the presence of clinical lymphdenopathy. It is reasonable to consider a selective neck dissection such as supra-omohyoid neck dissection in situations where there is uncertainty regarding the pathological diagnosis in the phase of clinically suspicious lymphdenopathy. Here we are presenting a case report of OVC in 65-year-old male patient treated with surgical excision and right side supra-omohyoid neck dissection.

ABSTRACT

Giant condyloma (Buschke-Loewenstein tumour) is a rare disease that commonly affects the urogenital area. We encountered an eighteen year old, nonhomosexual, HIV negative, male, who presented with a cauliflower like warty mass around perianal region. The mass was 8×6 cm in size, circumferential, soft and papillary in nature. Wide excision of mass was carried out with anoplasty reconstruction. Post-operatively, wound healing was uneventful. After wound healing, the topical applications of five percent imiquimod were done regularly under the care of dermatologist for three months. At six months of follow-up, patient is free from tumour, and is having neither signs of anal stenosis nor incontinence.

Introduction : Les carcinomes verruqueux du larynx sont des tumeurs rares. Ils posent des problemes diagnostics et therapeutiques. Le but de notre travail est de detailler les difficultes diagnostiques et therapeutiques du carcinome verruqueux du larynx. Patients et methodes : Treize malades ont ete traite d'un carcinomes verruqueux du larynx entre 1992et 2007. Resultats : Une chirurgie a ete pratiquee pour dix patients dont 7 ont eu un curage ganglionnaire. Deux patients etaient traites par radiotherapie exclusive et un autre par radio chimiotherapie concomitante. L'evolution etait bonne sans recidive ni metastases chez tous les patients qui ont ete traites chirurgicalement avec un recul moyen de 37 mois. Conclusion : Le carcinome verruqueux du larynx est une forme rare; de bas grade et bien differenciee du carcinome epidermoide. Il s'agit d'une tumeur a croissance lente et localement agressive dont le traitement de choix est l'exerese chirurgicale en marges saines

ABSTRACT

El carcinoma verrucoso (CV) es una variante rara del carcinoma de células escamosas con características morfológicas y comportamiento específico. El presente estudio relata el caso de una paciente de género femenino, de 68 años de edad, que presenta un carcinoma verrucoso en lengua, indoloro y con 8 meses de evolución. Además, se realizó una breve revisión de casos clínicos del Instituto de Referencia en Patología Oral (IREPO) de la Facultad de Odontología de la Universidad de Chile, diagnosticados entre enero de 1984 y octubre de 2010, encontrándose 20 casos, con un promedio de edad de 70 años, localizados con mayor frecuencia en encía inferior y lengua.


Verrucous carcinoma (VC), a rare variant of squamous cell carcinoma is an established entity with distinctive morphology and specific clinical behavior. The present study describe a case report of a 68-year-old women who presented a tongue verrucous carcinoma, asymptomatic, that had about 8 months of evolution. A brief review of VC cases diagnosed in Oral Pathology Referral Institute (IREPO), Faculty of Odontology, University of Chile, between 1984 and 2010. It was found 20 cases of verrucous carcinoma with a median age of70-years-old, the most common places were lower gingiva and tongue.

The tumor of buschke lowenstein or giant condyloma acuminata is a pseudo-epitheliomatous profileration bilonging to verrucous carcinoma group due to their aggressive local evolution.A malignant change as mic invasive carcinoma or epidermoiz keratinizing carcinoma well-differentiated was reported.Diagnosis and surgical treatment represents an efficient alternative at the moment that must be precocius and large due to the frequencies of local recurrences and malignant change. We present in this stady an observation of an unusual localization of buschke Lowenstein tumor with review of literature 50 year-old men presented with bilateral tumours it the battock area whose diagnosis of buschke Lowenstein tumour h been clinically suspected and confirmed by biopsy and histopathologic study .The cover has been achieved by VY fasciocutaneous flaps of the muscle gluteus maximus

ABSTRACT

El carcinoma más común en las vías aéreo-digestivas superiores (VADS) es el carcinoma de células escamosas (>90%). Es difícil precisar la prevalencia de las denominadas variantes del carcinoma de células escamosas (CCE). El diagnóstico diferencial es necesario dadas sus diferencias no sólo en el manejo sino también en su pronóstico. El carcinoma verrucoso es una de las variantes del carcinoma de células escamosas bien diferenciado no metastizante, caracterizado por ser una neoplasia de un crecimiento lento, de patrón exofítico verrucoso. El rol de la radioterapia (RDT) en el tratamiento del carcinoma verrucoso es motivo de discusión por su posible transformación en carcinoma anaplásico. A través de la siguiente revisión bibliográfica se busca aclarar dicha controversia.


Squamous cell carcinoma is the most common upper aerodigestive tract (UAT) carcinoma (>90%). Precising the prevalence of the so-called varieties of squamous cell carcinoma has proved difficult. Differential diagnosis is necessary, on account of the differences between varieties both in management and prognosis. One of such varieties, verrucous carcinoma of the larynx, is a well-defined, no-metastatic tumor, characterized as a slow-growth neoplasm, of verrucous exophyitic pattern. Since laryngeal verrucous cancer may transform into anaplastic carcinoma, the role of radiotherapy in its treatment has been discussed. The aim of the present literature review was to shed light on this controversy.

ABSTRACT

El carcinoma verrugoso, tumor de Ackerman o papilomatosis florida, es una variedad bien diferenciada del carcinoma de células escamosas. Fue descrito por primera vez en 1948 por Lauren Ackerman (Francia), el que observó que dicho tumor presentaba apariencia morfológica característica y un comportamiento clínico específico, por lo que debía ser separado de los otros carcinomas de células escamosas. Se lo atribuye al consumo del tabaco en sus diferentes formas, la responsabilidad en la aparición de esta lesión, sumándose otros factores irritntes crónicos locales. Actualmente se le otorga gran importancia al VPH (virus papiloma humano), principalmente al tipo 16, por estar presente en el 98 por ciento de las lesiones; queda por dilucidar si tiene función pirmaria en la etiología de la lesión o es un agregado posterior. Presentamos un caso de carcinoma verrugoso de Ackerman en la mucosa bucal, en un paciente varón de 51 años con historia de consumo de tabaco, alcohol y coqueo con bicarbonato de sodio durante larga data. Se describen las manifestaciones clínicas, histopatológicas y el control posterior a los 5 años.

ABSTRACT

El carcinoma verrucoso corresponde a una variante del carcinoma escamoso bien diferenciado. En el tracto genital femenino la región más comprometida es la vulva y raramente se presenta en el cuello uterino. Desde su descubrimiento ha presentado una serie de denominaciones, lo que ha dificultado su estudio. Se caracteriza por ser de lento crecimiento e invadir localmente casi sin metastizar a distancia. Su tratamiento es quirúrgico, siendo radiorresistente. Se plantea el diagnóstico diferencial con el carcinoma papilar de células escamosas y la condilomatosis cervical. En la literatura mundial se describen no más de 33 casos en esta localización, por lo que se expone nuestra experiencia además de compararla con la información disponible al respecto

ABSTRACT

El carcinoma verrugoso o tumor de Ackerman es considerado una variedad especial del carcinoma espinocelular bien diferenciado que se caracteriza por su bajo grado de malignidad, comportamiento clínico específico y un pronóstico favorable. Este tumor, asociado desde siempre con el consumo de tabaco masticable, higiene bucal deficiente y prótesis mal adpatadas, se ha visto relacionado últimamente con la infección por el virus de papiloma humano. Presentamos un caso de carcinoma verrugoso en mucosa de cavidad bucal con historia de prótesis mal adaptadas y consumo de tabaco masticado en una mujer blanca de 60 años. Puesto que el diagnóstico de esta lesión suele ser confundido, el examen clínico e histopatológico del carcinoma verrugoso son indispensables, ya que de ellos depende la identificación del tumor y posterior planificación del tratamiento
